Waiver of Liability
I acknowledge that participation in the Roots and Revival Dance program involves physical activity, including but not limited to dance exercises, stretching, and other movements. I understand that such activities carry inherent risks, including the potential for injury.
By signing below, I agree to the following:
I voluntarily allow my daughter to participate in the Roots and Revival Dance program and accept full responsibility for any risks associated with her participation.
I hereby release, waive, and discharge Kristen and all affiliated instructors, staff, volunteers, and representatives from any and all liability, claims, demands, or causes of action arising out of or related to any injury, loss, or damage that may occur during the program.
I agree to inform the instructors of any relevant health conditions, injuries, or physical limitations that may affect my daughter’s ability to participate safely in the program.
